Best review of CoQ10 supplementation and heart failure?

To date (autumn 2026), the best meta-analysis of studies of CoQ10 supplementation and heart failure is the 2024 review by Xu et al.

Xu et al pooled the data from 33 randomized controlled trials of CoQ10 in heart failure. They reported that supplementary CoQ10 is associated with lower all-cause mortality (36% relative reduction) and with fewer heart-failure hospitalizations (50% relative reduction). In addition, they found the following health benefits:

  • modestly improved left-ventricular ejection fraction
  • improved 6-minute walking distance
  • improved NYHA functional class
  • improved BNP heart failure biomarker levels

However, Xu et al did note that the clinical evidence quality varied. Accordingly, the authors have called for larger, longer trials [Xu 2024].

Best review of CoQ10 supplementation and antioxidant activity?

In a 2026 meta-analysis of 14 RCTs enrolling 433 study participants, Zhang et al reported that oral Coenzyme Q10 supplements elevated blood Coenzyme Q10 concentrations and reduced blood malondialdehyde concentrations. Specifically, the authors identified blood malondialdehyde concentrations as an oxidative-stress biomarker [Zhang 2026].

In a 2022 review of 34 RCTs comprising 2,012 study participants, Dai et al found that taking 100–150 mg/day Coenzyme Q10 supplements was effective at attenuating oxidative stress status in adults [Dai 2022].

In a 2021 meta-analysis of 17 RCTs enrolling 972 study participants, Hajiluian et al reported that CoQ10 supplements elevate antioxidant enzyme levels in the body and have positive effects on antioxidant defense against oxidizing agents. However, they warned that the clinical evidence is limited in scope [Hajiluian 2021].

Best review of CoQ10 supplementation and anti-inflammation activity?

In a 2023 review, Hou et al analyzed the pooled data from 31 RCTs enrolling 1,517 study participants. They found significant reductions in the inflammatory biomarkers C-RP, IL-6, and TNF-α in the general population. Consequently, they proposed daily supplementation of 300-400 mg Coenzyme Q10 for inhibition of inflammatory factors [Hou 2023].

Best review of Coenzyme Q10 supplementation and blood pressure?

Zhao et al reported an analysis of 26 studies with 1,831 study participants. Systolic blood pressure fell, on average, by approximately 4.8 mmHg, with the strongest effect of CoQ10 supplementation around 100–200 mg/day. Studies of longer duration showed better results [Zhao 2022].

Best review of CoQ10 supplementation and diabetes/metabolic disorders?

In a 2026 review, Zhang et al included 64 RCTs enrolling 3,422 study participants. Significant reductions in HbA1c, fasting glucose, fasting insulin, HOMA-IR, triglycerides, LDL-C, total cholesterol, CRP, TNF-α, and IL-6 correlated with CoQ10 supplementation [Zhang 2026].

Also in 2026, Musazadeh et al conducted an umbrella review of eight meta-analyses. The pooled analysis demonstrated that supplementary Coenzyme Q10 may have a moderate beneficial effect on glycemic control in diabetic patients [Musazadeh 2026].

Best review of CoQ10 supplementation and endothelial function?

In 2024, Daei et al analyzed the pooled data from 12 studies comprising 489 study participants. The study results demonstrated significant increases in Flow Mediated Dilation (FDM) after CoQ10 supplementation. Furthermore, changes in FMD percent were associated with the dose of the supplementation [Daei 2024].

Note: FMD is a surrogate marker of endothelial function; thus, Coenzyme Q10 supplementation improved endothelial function as assessed by FMD.

Conclusion: Clinical Evidence for CoQ10 Supplementation

Antioxidant activity: fairly consistent biomarker evidence.

Anti-inflammation activity: modest reduction in C-RP/IL-6/TNF-α biomarkers.

Blood pressure: modest reductions in systolic blood pressure.

Diabetes/metabolic disorders: modest but positive evidence.

Endothelial function: promising results based on surrogate FMD outcomes.

Heart failure: lower mortality and fewer hospitalizations.
