What are beneficial outcomes of supplementary CoQ10 for women trying to get pregnant?  Studies of the relationship between CoQ10 and female fertility show promising results [Jiang 2025]:

  • CoQ10 supports healthy development of oocytes.
  • CoQ10 prevents premature ovarian failure.
  • CoQ10 increases ovarian sensitivity to estrogen and improves ovarian function.
  • CoQ10 improves the quality of eggs during IVF treatment.

How Is Coenzyme Q10 Important to Female Fertility?

What does CoQ10 do, exactly? Jiang et al [2025] list the following important biological activities of CoQ10:

  • CoQ10 is an essential part of cellular mitochondrial ATP energy generation. Good oocyte development and maturation depend on optimal mitochondrial ATP.
  • CoQ10 is a potent lipid-soluble antioxidant. It scavenges harmful free radicals in cells and protects cells against oxidative damage. Oocytes are sensitive to free radical damage. Oxidative stress contributes to female infertility.
  • CoQ10 facilitates the activity and functionality of immune cells. The beneficial effect of CoQ10 on the immune status of female infertility patients may be important.
  • CoQ10 supplementation will most likely be necessary for women in their 30s and beyond. Typically, the body’s bio-synthesis of CoQ10 diminishes with increasing age above the age of 25 years [Kalén 1989]. Mitochondrial dysfunction is characteristic in women with diminished ovarian reserve.

Is CoQ10 Supplementation Associated with Improved Oocyte Quality?

In their 2025 review, Jiang et al have summarized the findings of numerous studies that show a beneficial effect of CoQ10 supplementation on oocyte quality. Better oocyte quality results in better binding of sperm to oocyte, in better embryonic development, and in better clinical pregnancy rates.

Jiang et al [2025] note that mitochondrial dysfunction and oxidative stress hinder oocyte maturation. CoQ10 is known for its role in mitochondrial function and in antioxidant protection.

A good example of what Jiang et al are reporting is the 2018 Giannubilo study of CoQ10 supplementation in women undergoing IVF-ET treatment. The study results show that CoQ10 supplementation improve follicular fluid oxidative metabolism, oocyte quality, and the rate of successful fertilization, especially in over 35-year-old women [Giannubilo 2018].

Does CoQ10 Supplementation Help Assisted Reproductive Technology?

Jiang et al cite a 2023 meta-analysis conducted by Zhu et al. The latter authors have analyzed the data from 16 randomized controlled trials enrolling a total of 2,323 women with poor ovarian response. The Zhu results show CoQ10 supplementation linked to increased pregnancy rates in both natural conception and assisted reproductive technology conception [Zhu 2023]. A handful of studies have now shown that CoQ10 supplementation improves oocyte quality and increases the clinical pregnancy rate in assisted reproductive technology [Jiang 2025].

What is CoQ10 Supplementation’s Effect on Ovarian Aging?

Jiang et al suggest that diminished CoQ10 expression, especially in women in their mid-30s and older, is linked to ovarian aging. Lower levels of CoQ10 are associated with decreased mitochondrial function and decreased energy production. This, in turn, is likely to have a negative impact on ovarian reserve. Some research indicates that CoQ10 supplementation at a dosage of 600 mg/day for at least 60 days can improve ovarian response in women with poor ovarian response.

Does CoQ10 Supplementation Affect Embryonic Development?

CoQ10 promotes better mitochondrial function and reduces oxidative stress. In so doing, the CoQ10 positively impacts egg quality and ovarian function. Moreover, the CoQ10 improves embryo quality and development [Jiang 2025].

Conclusion: CoQ10 Supplementation and Female Fertility

CoQ10 supplementation is well-tolerated and safe. It has practically no toxicity and has no potential genetic toxicity. It does not cause serious adverse reactions in humans [Jiang 2025].

Women experiencing premature ovarian failure and women aged 35 years and older can benefit from long-term CoQ10 supplementation of CoQ10 [Jiang 2025].

Clinical trials have typically used daily doses of 200 to 600 mg/day [Jiang 2025].

Because of their different formulations, all CoQ10 supplements are not equally effective. It is important to buy a CoQ10 supplement with documented bioavailability and with documented clinical efficacy.
